Zum nächstmöglichen Zeitpunkt suchen wir ⦠Pflegefachpersonen (m/w/d) für die Pflegebegutachtung In den ersten Monaten der Einarbeitung ist eine Beschäftigung in Vollzeit (38,5 Wochenstunden) bzw. in Teilzeit mit mindestens 80 % erforderlich. Nach Abschluss der Basiseinarbeitung bieten wir auch verschiedene Teilzeitmodelle mit einem Beschäftigungsumfang von mindestens 60 % an. Ãberwiegende Einsatzgebiete sind die Landkreise Schwarzwald-Baar-Kreis, Tuttlingen und Rottweil. Voraussetzung ist ein Wohnort innerhalb des Einsatzgebiets. Ihre Aufgabe bei uns: Einzelfallbegutachtung auf Grundlage des SGB XI in häuslicher Umgebung, in Einrichtungen oder nach Aktenlage, einschlieÃlich der Empfehlung von Leistungen, z. B. zur medizinischen Rehabilitation, zu wohnumfeldverbessernden MaÃnahmen sowie zu Hilfsmitteln.
